University of Chicago Medicine Ingalls Memorial conducts a Community Health Needs Assessment (CHNA) on a regular basis and develops a strategic implementation plan to improve the health of residents in our 13-ZIP-code Southland primary service area. The CHNA offers a systematic, data-driven approach to determining the health status, behaviors and needs of the residents in our community benefit primary service area.

UChicago Medicine Ingalls Memorial Community Health Needs Assessment

Required by the Affordable Care Act, a Community Health Needs Assessment enables communities to identify issues of greatest concern and decide how to allocate resources to address those issues, thereby making the greatest possible impact on community health status.
