UCM Care Network Affiliated Providers

We're really committed to not only the growth of our Clinically Integrated Network, but to the delivery and the success of both our providers as well as the patients.

The Clinical Integrated Network is necessary as far as how health care is going to move forward with the change in landscape. Ingalls Provider Group and now the Clinically Integrated Network have really become a high performing shared savings team in the Chicagoland area. We've seen some rapid growth and really good reimbursement from our work.

We recognize that even though it's already outstanding care that's provided in our affiliated hospitals and in practices that join our care network, the reality is we know that you can't do it alone.

Primary care physicians need support. Nurses in the clinics need support. We're not just here to come in and solve a problem. We're here to work with you.

When we think about expanding the network, we're really looking at quality. Yes, there's an important element of geography. We want to be sensitive to where we can reach out and where we can do the most good for patients and communities. We have a deep understanding of the needs in those communities, both clinically and otherwise, and we try to mirror that in the geography.

I think a big goal for the Clinical Integrated Network is to continue to expand into the community and provide these services and really the high level care that we've expected and we've seen at Hyde Park for years. Bringing that level of care to all of our patients in the community is really a game changer for us.

We pride ourselves in how we go into the primary care center work in the clinics, get to know our primary care teams, providers, nurses, MAs to support the patient.

My responsibility and my role is to reach out to all our Medicare patients, to schedule their yearly checkups.

What we do is spend some time working with their team, their doctors, their specialists, and also seeking out any community resources that might come into play to address those needs.

We do an assessment for the patients. We call them and outreach to them over the phone. After that, we do a brief assessment, and we go down a list to see what are the needs that the patients have? We look at those assessments to see what the patient needs, and then we'll try to get those resources to the patient.

Releasing the metrics and the quality of care that we're providing to these patients exceed the levels that we would have seen even a year or two ago.

Our Clinically Integrated Network is the highest performing academic network in the Chicagoland area. And that's a commitment that we make to our patients. We want them to know that when they work with a provider who represents our network, that they're going to get outstanding care. They're going to get the outcomes they deserve. And yes. That also means that the providers and the practices taking part in the network will share in the rewards that we receive for that outstanding and excellent care.

We give the best service that we can possibly give not only to our patients but also our doctors.

We've made and continue to make significant investments in building out resources around population health, care coordination, recognizing that we want our physicians to be able to focus on what they do uniquely well.

Our team is passionate in what we do. I see it every day. I think we are here for a reason, and we all love the job that we do.
