Create a MyChart Account
To sign-up for access, visit the MyChart patient portal. You'll need the activation code located on your billing statement or your after-visit summary. This code will enable you to log in and create your own username and password.
If you do not have a MyChart activation code, you can activate your account through our secure online sign up process. Start by filling in your demographic information (name, address, etc...) and then we will verify your identity by asking you a few questions from a secure third-party verification system. Once your identity has been verified, you will be able to create your MyChart account. If you have questions or need help, please contact us at 1-844-442-4278.
For your security, your activation code expires after 30 days and is no longer valid after the first time you use it. If you need a new activation code, you can call us at 1-844-442-4278.
Once you have created your username and password, you may go download the MyChart Mobile application for your iPhone or Android via the Apple store or Google Play store.
Access a Family Member's Health Records
You are also able to access your family member’s health records on MyChart through proxy access.
For access to a child’s chart, fill out the proxy access request form on our MyChart home page. You can also ask about this at your child’s next visit.
To allow someone access to your account or to access another adult’s account, patients are able to invite someone to proxy access to their MyChart if they are over the age of 17. This is done directly in MyChart and does not require a paper form.

MyChart Frequently Asked Questions
Patients who want to take part will get a MyChart activation code during their clinic visit. The code will allow you to log in and create your own username and password.
If you were not given an activation code, you can use the “Request Access to My Account” button and fill out the request form on the MyChart home page. You will get an activation code by email within 7 business days.
MyChart is a free service that offers patients a personal and secure online access to parts of their medical records. It lets you securely use the Internet to help manage and get information about your health. With MyChart, you can use the Internet to:
- Schedule an appointment using the online scheduling tool
- Schedule a video visit
- Ask for prescription renewals
- See test results
- Access trusted health information resources
- See your health summary from the MyChart electronic health record
- Securely talk on-line with your medical care team
Your test results are released to your MyChart account after your doctor has first looked them over. This is often within 1 to 7 business days after your doctor gets the final tests result.
Some tests results may not be able to be seen in MyChart because of their very private nature. Most test results such as labs, radiology and pathology will be released to MyChart within 7 business days of receipt by your provider as well as historical results since 2013. If you have questions about a test result, use the “Get Medical Advice” request to send a message to your provider and team.
If you see any health information on MyChart that is not correct contact your doctor. Your MyChart information comes directly from your electronic medical record at your doctor’s office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.
You will generally receive an answer within 2-3 business days from your provider or their clinical staff.
Do not use MyChart for an emergency. If you have an emergency call 911.
You are also able to see your family member’s health records on MyChart. This is called Proxy access. It allows access to a child or adult UCM patient’s record.
We have two ways to sign up for proxy access:
- For access to a child’s chart fill out the “Request Proxy Access to My Child’s Account” form on our MyChart home page. You can also ask about this at your child’s next visit.
- To allow someone access to your account or to access another adult’s account, patients are able to invite someone to proxy access to their MyChart if they are over the age of 17. This is done directly in MyChart and does not require a paper form.
You are not able to ask questions on MyChart about another person’s information. This is because that information would then be in your health record and not in the correct health record of the other person. This could put the other person’s medical care at risk.
Married couples cannot share one MyChart account. Each adult must have their own account. This is because of the very private nature of medical information.
The legal parents or guardians can have proxy access to parts of their minor child's medical record online.
Only a child that has been seen by a doctor at The University of Chicago Medicine will have a MyChart account. Children under the age of 18 are not permitted to have access to their own individual MyChart account.
According to UCMC policy, proxy access will change to a limited version on the child's 12th birthday and will end on the child's 18th birthday.
- For children birth to 12 Years of Age: Parents and legal guardians are provided with full access to their child's MyChart account. A week before your child's 12th birthday, you will get an email letting you know that your access will be changed to a limited proxy access the day before your child's birthday.
- For Children 12 to 17 Years of Age: Parents and legal guardians are given limited access to their child's MyChart account. A week before the child's 18th birthday, you will get an email letting you know that access will end the day before your child's birthday.
If you forget your password click the "Forgot Password" link on the sign-in page. You can reset your password online by answering a few security questions. If you are not able to change your password, you may contact us at 1-844-442-4278 to ask for a new, secure password.
To update your personal information (including home address, e-mail or phone number) Log into MyChart. Go to ‘Personal Information’ from the menu. Address changes will first be verified and then updated by UChicago Medicine staff. Your mobile phone and email address will automatically be updated when you enter them in your record.
To change your password. Log into MyChart. Go to ‘Security Settings’ and following the steps to change your password.
You need access to the Internet and an up-to-date browser (such as Internet Explorer). When you have created your username and password, you can download the MyChart Mobile App for your iPhone or Android from the Apple store or Google Play store. However, you cannot create your account with the mobile app. You can only create an account with a web browser session.
Please note: We aim to protect your privacy and security of your information. When logged into MyChart, if you are not using your keyboard for a long time you will be automatically logged out of MyChart. If you need to leave your computer for even a short period of time, we recommend that you log out of your MyChart session.
To activate your MyChart account, you may use an activation code or link provided during a clinic visit or sent to your email or phone. If you haven't received one, you can sign up directly on the MyChart homepage. Click "Sign Up" and select "Sign up now." You'll be asked a few questions to confirm your identity.
